Job summary

Adoration Health in Vienna, WV is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in a home setting. You will work full-time with opportunities for PRN shifts, contributing to comprehensive care plans and direct nursing support in patients’ homes.

The role requires an RN license with CPR certification and at least 1 year of nursing experience, with a preference for home health or geriatric care. A 10k sign-on bonus is available for external candidates.

Qualifications

• RN with current, unrestricted license in the state of practice and residence
• Minimum 1 year nursing experience in patient care; home health/geriatrics preferred
• Valid driver's license and proof of car insurance per policy
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with MD orders and patient goals
• Direct nursing care: medications, treatments, interventions
• Provide pain and symptom management
• Educate and support patients and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate documentation
• Participate in on-call rotation as required by the local branch

Responsibilities

As a Home Health Registered Nurse, You will:

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans that align with MD orders and the patient's goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with an interdisciplinary team

• Participate in on-call rotation as required by the local branch

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
