Overview

The Meadows of Central Massachusetts is an 82‑bed transitional care facility located alongside Vibra Hospital of Western Massachusetts – Central Campus. The facility provides comprehensive skilled nursing and rehabilitation services for individuals in need of specialized medical care before they can return home or to another setting. Many of our patients have suffered serious injuries or illnesses and require day‑to‑day help while working toward recovery and regaining independence. Our approach incorporates therapeutic intervention and medical management.

8 Hours Shifts

Responsibilities

Responsible for the comprehensive assessment of residents and family needs to determine required nursing services; understands and integrates the needs of adult and geriatric residents in the delivery of nursing care. Provision of care includes direct care and delegation/supervision of all nursing care. Duties include demonstrating clinical competence, managing resident care, improving quality of care, establishing professional relationships, utilizing professional skills, fulfilling facility responsibilities, and developing clinical expertise. Manage and provide direct care for assigned residents and monitor quality of care during shifts. Serve as liaison to keep the Program Manager and/or Nursing Supervisor aware of major occurrences and needs.

Required Skills

• Current, valid, and active license to practice as a Registered Nurse in the state of employment.
• Current BLS certification from a Vibra‑approved vendor.

Additional Qualifications / Skills

• One (1) year of medical/surgical, rehabilitation, or skilled nursing experience preferred.
• Ability to project a professional image.
• Knowledge of regulatory standards and compliance requirements.
• Strong organisational, prioritising and analytical skills.
