Qualifications

Responsibilities

Conduct comprehensive assessments across physiological, psychological, developmental and spiritual factors.
Develop and implement individualized care plans and revise as needed.
Administer medications and treatments per orders and monitor for adverse effects.
Educate patients and families on conditions, treatments and self-care.
Coordinate with medical staff to optimize patient care and teamwork.
Document patient information and outcomes accurately in the medical record.
