Job summary

Allay Hospice is seeking a passionate Hospice RN Case Manager to provide expert, patient-centered care in our Brookfield, WI service area. You will work 1:1 with patients, coordinating with an interdisciplinary team to deliver compassionate hospice care.

The role requires RN licensure in the state, 1+ year of nursing experience (3+ preferred), and willingness to travel primarily across Waukesha and Milwaukee Counties. Competitive benefits and a 5,000 sign-on bonus are offered.

Qualifications

• Graduate of an accredited nursing program with current RN license in state of operation.
• One year nursing experience required; three+ years preferred, hospice experience a plus.
• Strong knowledge of hospice philosophy and care planning.
• Must have reliable vehicle and valid driver's license.

Responsibilities

• Assess/monitor physical, emotional, and clinical needs of hospice patients.
• Develop and implement individualized hospice care plans aligned with patient wishes.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management and educate families.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the branch.
