Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated RN Case Manager for Hospice to join our Parkersburg, WV team. You will deliver compassionate, patient-centered hospice care, assess needs, and collaborate with an interdisciplinary team to create and implement care plans.

Responsibilities include administering medications, pain and symptom control, and educating families.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.

• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.

• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Office Location: Parkersburg, WV

Coverage area: Wood, Wirt, Calhoun, Pleasants, Ritchie, and Roane counties

Schedule: Monday thru Friday, 8am - 5pm
