Grane Hospice Care, an affiliate of BrightSpring Health Services, seeks a compassionate RN Case Manager in York, PA to provide patient-centered hospice care in home settings. You will assess needs, develop care plans, manage medications, and educate families while collaborating with an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an insured automobile in good working order

Responsibilities

• Assess and monitor the physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients’ wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on‑call rotation as required by the local branch
