Job summary

Grane Hospice is seeking a passionate Hospice Admissions RN to join the Camp Hill, PA team. You will visit patients in homes or facilities, assess eligibility, and coordinate the start of hospice services with nursing, aides, and equipment.

Applicants should have RN licensure and hospice or related care experience, with a strong focus on patient-centered care and collaborative practice within an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the operating state.
• Hospice experience preferred or 3+ years in long-term care/ICU/ED in lieu of hospice.
• Valid driver's license with reliable vehicle and insurance as required.

Responsibilities

• Visit patients in homes, hospitals, or facilities referred for hospice care.
• Explain hospice services, discuss goals with patients and families, obtain consents and required documents.
• Obtain medical history, current meds, and symptoms; perform comprehensive assessments for eligibility.

As a Hospice Admissions RN You will

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptomsPerform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
