Office Location: 115 Union Avenue Altoona, PA 16602

Coverage area: Huntingdon County - may travel throughout the territory

Schedule: Monday-Friday, 8AM-4:30PM

Role Responsibilities
  • Assess and monitor physical, emotional, and psychological needs of patients
  • Create hospice care plans aligned with patient wishes and goals
  • Administer medications, treatments, and interventions
  • Manage pain and symptoms effectively
  • Educate and support patients' families and caregivers
  • Collaborate with interdisciplinary team members
  • Maintain accurate and timely documentation
  • Participate in on-call rotations as required

Qualifications
  • Graduate of an accredited nursing school with current licensure in the state of operation
  • Minimum one year of nursing experience; 3+ years preferred
  • Experience in hospice or similar setting preferred
  • Knowledge of hospice philosophy of care
  • Commitment to clinical and documentation excellence
  • Valid driver's license with insured, well-maintained vehicle
