Job summary

Phoenix Home Care & Hospice, based in Jefferson City, MO, is seeking a Registered Nurse (RN) to join as a Home Health Case Manager. The role offers a $5,000 sign-on bonus and a generous pay range of $65,000–$90,000 per year, with full-time and PRN options.

The Home Health Case Manager will oversee skilled nursing visits, care coordination, and collaboration with a PT/OT/SLP team to deliver patient-centered care in patients’ homes across Jefferson City and surrounding Missouri communities.

Qualifications

• Missouri RN license or Compact RN license.
• Valid driver’s license.
• Reliable vehicle with current auto insurance.
• Home health nursing experience preferred.
• Critical care experience preferred.
• Strong assessment, communication and clinical decision-making skills.
• Ability to work independently while collaborating with an interdisciplinary care team.

Responsibilities

• Provide skilled nursing visits and starts of care while coordinating a patient’s overall plan of care.
• Manage an assigned home health nursing caseload.
• Supervise LPN visits and coordinate care among healthcare providers.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Communicate with physicians to obtain orders and establish diagnosis-driven plans of care.
• Coordinate with insurance companies and other healthcare professionals.
• Educate patients and families about CHF, COPD, diabetes, wound care and medication management.
• Collaborate with Physical Therapy, Occupational Therapy and Speech Therapy.
• Provide hands-on patient care in patients’ homes.
• Use clinical judgment and diagnostic information to support quality outcomes.

What You’ll Do

• Educate patients and families about conditions including CHF, COPD, diabetes, wound care and medication management
• Collaborate with Physical Therapy, Occupational Therapy and Speech Therapy
• Provide hands-on patient care in patients' homes
• Use clinical judgment, physical assessment skills and diagnostic information to support quality patient outcomes

What We’re Looking For
