Overview

The Home Health Admission Nurse is a licensed professional who performs admissions and OASIS assessments while also administering skilled nursing care in the home to clients requiring professional nursing service. These services are performed in collaboration with the other members of the home health multidisciplinary team, in accordance with the physician's orders and under the direction and supervision of the Clinical Supervisor.

Responsibilities

• Assessing and admitting eligible patients for home health care services.
• Evaluation, development and implementation of plan of care.
• Skilled Nursing Care and Comprehensive Assessment.
• Teaching, Supervision, and Counseling Clients and Caregivers.
• Interdisciplinary collaboration.
• Problem solving and critical thinking skills.
• Oasis documentation.
• Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)
• Other duties as assigned.
  
  

Qualifications

• This position is 10-hour days 4 days a week with a weekend a month and two holidays a year obligation.
• Must have a minimum of 1 year of recent Home Health experience
