Responsibilities

• Provide direct patient care to a diverse population in post-acute and long-term care

• Administer medications, treatments, and therapies as prescribed
• Collaborate with patients, families, and providers to ensure continuity of care
• Document patient progress, medications, and treatments accurately
• Participate in quality improvement initiatives to enhance outcomes and satisfaction

Responsibilities:

• Provide direct patient care to a diverse population, including post-acute rehabilitation and long-term skilled nursing care
• Develop and implement individualized care plans, working closely with interdisciplinary teams to ensure comprehensive care
• Conduct initial and ongoing assessments, identify trends and patterns, and make recommendations for changes to care plans
• Administer medications, treatments, and therapies as prescribed by physicians
• Collaborate with patients, families, and healthcare providers to ensure continuity of care
• Document patient progress, medication administration, and treatments accurately and efficiently
• Participate in quality improvement initiatives and performance improvement projects to enhance patient outcomes and satisfaction
