Provide direct patient care to a diverse population, including post-acute rehabilitation and long-term skilled nursing care
Develop and implement individualized care plans, working closely with interdisciplinary teams to ensure comprehensive care
Conduct initial and ongoing assessments, identify trends and patterns, and make recommendations for changes to care plans
Administer medications, treatments, and therapies as prescribed by physicians
Collaborate with patients, families, and healthcare providers to ensure continuity of care
Document patient progress, medication administration, and treatments accurately and efficiently
Participate in quality improvement initiatives and performance improvement projects to enhance patient outcomes and satisfaction
