You will perform resident assessments, develop care plans with healthcare professionals, coordinate with the IDT, supervise CNAs, and ensure compliance with IDPH/CMS standards.

Qualifications

• Active Illinois Registered Nurse license is required.
• Strong, effective communication is essential.
• Exemplary leadership skills are expected.
• Long Term Care (LTC) experience preferred.
