Job summary

NHC HomeCare Lawrenceburg is seeking a Registered Nurse for full-time field work in Lawrenceburg, TN. The role centers on delivering patient-centered home care, coordinating with an interdisciplinary team to meet health goals.

Qualifications

• Unencumbered RN license in the state where the agency operates or compact state.
• Minimum one year experience as an RN.
• Home care experience is desirable.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assesses factors that affect health to develop a comprehensive care plan with desired outcomes.
• Collaborates with the interdisciplinary team to provide personal, medical and rehabilitation care.
• Educates patient and caregiver on health care and disease management and documents needs.

Position: Registered Nurse - NHC HomeCare Lawrenceburg Full Time

Licensure:

• Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.
• Minimum one (1) year experience as a RN / professional nurse.
• Experience in home care is desirable.

Position Highlights:

• Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.
• Assesses the physical, psychosocial, and environmental factors that affect a patient's health to develop a comprehensive nursing care plan which will attain the patients desired health outcomes in a culturally comfortable way.
• Collaborates with the interdisciplinary team to assure personal care, medical care and rehabilitation provide for optimal patient health and well-being.
• Teaches patient/caregiver in various aspects of health care and disease management appropriate to needs/level of education and understanding and documents continuing needs and levels of patients/caregivers understanding.
