Job summary

NHC HomeCare Pulaski is seeking an RN, Registered Nurse, to join our Pulaski HomeCare team. The role focuses on delivering comprehensive patient care in the home setting, identifying patient goals, and coordinating with the care team to promote recovery and well-being.

Ideal candidates hold an unencumbered RN license in the state of practice, with at least 1 year of RN experience; home care experience is desirable. This position emphasizes patient education and family engagement in care plans.

Qualifications

• Licensure: Unencumbered RN license in state of practice.
• Minimum one year RN experience.
• Home care experience desirable.

Responsibilities

• Uses the nursing process to identify and achieve patient goals (assessment, planning, implementation, evaluation).
• Assesses physical, psychosocial, and environmental factors to develop a comprehensive nursing care plan.
• Collaborates with the interdisciplinary team to ensure personal care, medical care and rehabilitation support optimal health outcomes.
• Educates patient and caregiver on health care and disease management and documents ongoing needs.

Licensure

• Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.


• Minimum one (1) year experience as a RN / professional nurse.


• Experience in home care is desirable.

Position Highlights

• Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.


• Assesses the physical, psychosocial, and environmental factors that affect a patient’s health to develop a comprehensive nursing care plan which will attain the patients desired health outcomes in a culturally comfortable way.


• Collaborates with the interdisciplinary team to assure personal care, medical care and rehabilitation provide for optimal patient health and well-being.


• Teaches patient/caregiver in various aspects of health care and disease management appropriate to needs/level of education and understanding and documents continuing needs and levels of patients/caregivers understanding.
