Job summary

Heritage Wellness & Rehabilitation in Independence, MO seeks a Registered Nurse (RN) for a day shift in a resident-centered skilled nursing and rehabilitation environment. The RN will provide high-quality clinical care, monitor conditions, supervise LPNs/CNAs, and collaborate with the interdisciplinary team to ensure safety and comfort.

Responsibilities include administering medications, performing assessments, coordinating care, and educating residents and families.

Qualifications

• Current Missouri RN license
• BLS/CPR Certification
• Experience in skilled nursing, rehabilitation, or post-acute care preferred
• Strong assessment, clinical judgment and critical-thinking skills
• Excellent communication and teamwork abilities
• Compassionate, dependable, resident-focused
• Ability to work on day shift nursing team

Responsibilities

• Provide skilled nursing care per physician orders and care plans
• Perform comprehensive resident assessments and identify condition changes
• Use clinical judgment to determine interventions
• Administer medications and treatments as ordered
• Provide and oversee wound care, IV therapy, and other skilled nursing treatments
• Communicate changes in resident condition to physicians and team
• Supervise and support LPNs and CNAs during the shift
• Respond to resident needs, call lights, and emergencies
• Complete accurate and timely clinical documentation
• Coordinate admissions, transfers, and discharges as needed
• Collaborate with physicians, residents, families, and the care team
• Educate residents and families regarding care and health needs
• Follow infection prevention protocols and participate in QAPI initiatives
• Maintain compliance with CMS/state regulations
• Promote dignity, safety, and quality of life
• Support a positive, team-oriented nursing environment

Position Summary

The Registered Nurse (RN) provides high-quality, resident-centered nursing care while using strong clinical judgment to assess residents, identify changes in condition, and coordinate appropriate interventions. The RN administers medications and treatments, communicates with physicians and families, supervises and supports LPNs and CNAs, and collaborates with the interdisciplinary team to promote positive resident outcomes.

• Coordinate admissions, transfers, and discharges as needed

• Collaborate with physicians, residents, families, and the interdisciplinary care team

• Educate residents and families regarding care, treatment, and health needs

• Follow infection prevention and infection control protocols

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives

• Maintain compliance with CMS, state, and facility regulations

• Promote resident dignity, safety, comfort, and quality of life

• Support a positive, respectful, and team-oriented nursing environment

Qualifications

• Current Missouri Registered Nurse (RN) license in good standing

• Current BLS/CPR Certification

• Skilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferred

• Strong assessment, clinical judgment, and critical-thinking skills

• Excellent communication and teamwork abilities

• Compassionate, dependable, and resident-focused

• Ability to work effectively as part of a day shift nursing team
