Job summary

Alternate Solutions Health Network is seeking an RN to provide skilled nursing care in patients' homes across the Avon area and surrounding communities. You will work autonomously, coordinate with physicians and a multidisciplinary team, and focus on delivering compassionate, high-quality care in the patient’s preferred environment.

The role requires an active RN license and at least one year of acute care experience; home health experience is preferred.

Qualifications

• RN with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care experience preferred.

Responsibilities

• Plan of Care: Observe and monitor patient conditions and update care plans.
• Minimize patient risk by administering medications safely and prioritizing high-risk visits.
• Ensure continuity of care through coordinated multi-disciplinary inputs.
• Educate patients and families about disease processes using teach-back methods.
• Comply with agency policies, Medicare guidelines, and proper documentation.
• Understand and follow state practice rules and communicate schedule changes to schedulers.
