Job summary

Heritage Park Care Center in Carbondale, CO, part of Life Care Centers of America, is seeking a qualified Registered Nurse to join our care team for short-term rehabilitation and long-term care.

The RN will assess patients, revise care plans, supervise CNAs and LPNs, and collaborate with an interdisciplinary team to maintain high-quality patient care. CPR certification is required, and an active RN license is essential for employment.

Qualifications

• Nursing diploma (associate’s or bachelor’s)
• Active RN license in good standing
• CPR certification required or obtained during orientation
• 1 year skilled nursing experience preferred

Responsibilities

• Implement and interpret department goals and policies
• Provide excellent patient care with a positive attitude
• Communicate effectively and use good judgment within an interdisciplinary team
• Evaluate patient needs through ongoing assessment and revise care plans as condition changes
• Provide clinical supervision to CNAs and LPNs on a daily basis

Education, Experience, And Licensure Requirements

• Nursing diploma (associate’s or bachelor’s degree in nursing)
• Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
• One (1) year skilled nursing experience preferred
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
