Job Highlights :

We are looking for an enthusiastic nurse who understands the challenges of Home Health and Hospice care but is committed to finding solutions. Your energy and passion will make a difference in the lives of our patients and their families.

Your Responsibilities Will Include :

  • Meeting with patients and families to discuss continuum of care services.
  • Communicating with referral sources such as case management, social work, clinics, and providers.
  • Coordinating care between various departments to ensure seamless service delivery.
  • Conducting eligibility and chart reviews to determine appropriateness for services.

Schedule & Compensation :

Enjoy a work schedule of approximately 8 AM to 5 PM with an on-call rotation every 10-12 weeks. We offer a $10,000 sign-on bonus for qualified candidates with a 24-month commitment, along with a comprehensive benefits package starting on your first day of the month after you begin!

Additional perks include night and weekend differentials and reimbursement of $0.70 per mile.

Job Summary :

As a vital member of our team, you will create a positive patient care experience by facilitating problem-solving and decision-making. You will ensure accuracy and timeliness in all functions related to insurance prior authorization, plan benefits, and medical necessity documentation.

Job Responsibilities and Requirements :

Primary Responsibilities :

  • Foster a welcoming environment for patients by following established guidelines.
  • Collaborate with physicians and multidisciplinary teams to ensure optimal care.
  • Maximize revenue capture while optimizing cost savings.
  • Apply evidence-based practices to nursing care, tailored to patient needs.
  • Demonstrate professional nursing standards and adhere to the ANA Code of Ethics.
  • Maintain a constant state of alertness and safety.
  • Perform other duties as assigned.

Education :

  • Graduate of an accredited school of nursing or equivalent education for licensing.

Experience :

  • No prior experience required.

Physical Requirements :

  • Frequent lifting / carrying and pushing / pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, and repetitive movements.
  • Frequent use of vision and hearing for effective communication.
  • Occasional lifting of objects weighing 25-50 lbs.

Required Professional License and Certifications :
