• Collect health history data from patients, families, and previous medical records, using effective communication principles tailored to adolescent, adult, or geriatric patients.
• Complete admission assessments within the acceptable time frame and notify consults when admitting criteria are identified.
• Develop and continually update individualized, age-appropriate plans of care, involving patients, families, and significant others to ensure safety and education.
• Document patient responses to nursing interventions, medications, treatments, and education, ensuring legibility, timeliness, and accuracy.
• Initiate and coordinate patient/family teaching using age-appropriate instructions and tools, documenting all educational activities.
• Participate in interdisciplinary patient care meetings, developing and implementing discharge plans from admission through hospital stay.
• Transcribe and sign off orders correctly, utilizing computer systems and clarifying any unclear orders prior to implementation.
• Apply telemetry leads for monitoring, ensuring correct alarm settings, and interpreting telemetry monitoring strips accurately.
• Assist with elective cardioversion and manage patients on high-risk intravenous cardiac drugs, understanding treatment implications.
