Qualifications

U.S. citizenship; VA policy applies; background check.
RN license in any US state; BSN preferred.
Experience in program management and leadership.

Responsibilities

Lead a facility-wide HBPC program and ensure quality of care.
Coordinate staff education, competencies, and quality improvement.
Monitor program data, performance measures, and patient satisfaction.
Collaborate with QSV, Medical Director, and interdisciplinary teams.
Schedule and oversee HBPC staff, visits, and staff meetings.
