Job Summary:

The PRN Registered Nurse will provide patients support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state and will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management.

Responsibilities:

  • Provides education to patients and their families about disease prevention, wellness maintenance, health promotion, and management of their condition.
  • Arranges follow-up appointments, ensure continuity of care, communicate with providers and clinic staff, track patient progress over time.
  • Handles prescription refill requests, administer medications (as per physician orders and clinic policy), monitor for drug interactions or side effects, ensure patient understanding of medications.
  • Performs clinical assessments, identify patient‐specific health needs, develop individualized care plans in collaboration with providers, and implement those plans.
  • Develops specialty knowledge in the clinic’s population (e.g. women’s health issues, obstetrics, gynecology), act as a resource to other clinical staff and providers.
  • Measures patient outcomes, assess how patients are doing relative to goals, make modifications to care plan or interventions when needed.
  • Works closely with physicians, nurse practitioners, medical assistants, and other clinic staff to deliver coordinated, patient‐centered care.
  • May perform other duties as assigned.

Qualifications:

  • Baccalaureate Degree, ADN Degree, or Diploma RN. Major: Registered Nurse
  • RN with a minimum of one (1) year of RN experience.
  • Current BLS certification.
  • Valid Arkansas Registered Nursing License

Preferred Qualifications:

  • ACLS certification or obtain certification within one year of hire date.
  • Knowledge or experience in PCMH desired.
  • Experience in an ambulatory care setting.
