Summary of Job Duties: The RN will provide direct patient care, coordinate care, triage, work with providers and staff, facilitate appointments, coordinate transitions of care, and educate patients on complex conditions. The RN will develop and implement individual health plans with patients, families, and providers.

Qualifications:

Minimum Qualifications:

• Bachelor’s, Associate’s, Diploma, or Master’s degree in Nursing
• Graduate of an accredited RN program
• Valid RN License
• At least 1 year of RN experience
• Basic computer skills
• Excellent communication skills
• Strong work ethic and accountability

Preferred Qualifications:

• Current BLS
• ACLS certification
• Scrubbing experience
• Orthopedic experience (a plus)

Responsibilities:

• Provide nursing care adhering to the Nurse Practice Act and institutional policies
• Assess, develop, implement, and evaluate patient care plans
• Administer medications and educate patients and families
• Support and mentor team members and students
• Perform other duties as assigned
