Reports to: Director of Clinical Management

National Church Residences Home Care is a Home Health & Hospice agency with a dedicated team of healthcare professionals. We are owned and operated by National Church Residences, the nation's largest provider of affordable senior housing and healthcare services.

We are seeking a Registered Nurse who shares our vision to improve living and care for seniors.

Essential Functions
  1. Develop, implement, and update individualized care plans, coordinating with physicians, clients, caregivers, and representatives.
  2. Provide education, mentoring, and support throughout the care plan, and communicate changes to all involved parties.
  3. Use health assessment data to identify problems, set goals, and determine interventions; coordinate with community health providers and interdisciplinary teams.
  4. Perform initial assessments, physical examinations, and history taking to determine home care needs.
  5. Initiate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed.
  6. Communicate with physicians regarding client needs, report changes, and update clinical notes accordingly.
  7. Provide direct client care within the scope of the State Nurse Practice Act.
  8. Educate and mentor clients, caregivers, and representatives; support end-of-life care discussions.
  9. Re-evaluate nursing needs regularly and revise care plans as needed.
  10. Coordinate discharge planning with interdisciplinary team members.
  11. Participate in interdisciplinary team meetings to ensure proper communication, documentation, and care updates.
  12. Supervise and evaluate LPNs/LVNs and aides, ensuring competency and adherence to policies.
