Job summary

Maxim Healthcare in San Francisco area seeks a Registered Nurse to deliver home-based care to pediatric and/or adult patients. The role includes applying the nursing process, planning and adapting care, and ensuring safe administration of medications in clients' homes.

Qualifications

• Current RN license for the practicing state.
• Current Health Certificate as required by facility or state.
• Current PPD or Chest X-Ray clearance.
• Current BLS certification.
• One year RN experience preferred.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate patient care at the client home.
• Initiates care plans and revises with prescriber input as patient needs change.
• Provides therapeutic, preventive, and rehabilitative nursing actions based on diagnoses.
• Responds to changes in patient condition and communicates with physician or supervisor.
• Administers medications and treatments, monitors effectiveness, documents results.
• Reviews care plans and coordinates education, discharge planning, and goal setting.

Salary: $50 - $55 / per hour

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Registered Nurse (RN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Registered Nurse (RN) experience preferred.
