Overview

Full-time RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location.

Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care.

Requirements

• Current MA RN License required with minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred.
• A minimum of one year experience working with a frail or elderly population.
• Prior experience in interdisciplinary team setting a plus.

Schedule

Monday - Friday, 8:00 AM - 5:00 PM and will include weekend/holiday/on-call rotation.
