Job summary

Crossover Health is seeking a Registered Nurse to join its team-based Primary Health model, delivering direct patient care and leading care coordination within an interdisciplinary team. The RN will participate in clinic huddles and implement enhanced care pathways across in-person and virtual settings.

The ideal candidate has a clinical background with at least 3 years of experience, an active RN license, and BLS certification.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum of 3 years of clinical experience.
• Current Registered Nurse license in the state you are practicing.
• Current BLS (Basic Life Support) certification.

Responsibilities

• Performs evidence-based nursing care and patient education.
• Performs care management by connecting patients with specialists and following up to ensure patients are getting the care they need.
• Utilizes population management tools to ensure patients receive optimal care based on industry-wide recommendations.
• Provides phlebotomy services, performs in-house laboratory tests and works with the provider to determine how and when to send out specialty labs.
• Screens for vaccine-preventable diseases and administers primary care and travel-related immunizations.
• Manages RN schedule and appropriateness of appointments; contacts patients when needed.
