Job summary

Crossover Health is seeking a Registered Nurse (RN) to support an exceptional patient experience within a collaborative care team. The RN provides direct patient care and demonstrates servant leadership while leading clinic-wide huddles and leveraging an interdisciplinary approach to integrated care.

This role requires a nursing degree, current RN license in the state, and BLS certification, with at least three years of clinical experience.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum 3 years of clinical experience.
• Current Registered Nurse license in the practicing state.
• Current BLS certification.

Responsibilities

• Performs evidence-based nursing care and patient education.
• Performs care management by connecting patients with specialists and following up.
• Utilizes population management tools to ensure optimal care.
• Provides phlebotomy services and in-house laboratory tests.
• Screens for vaccine-preventable diseases and administers immunizations.
• Manages RN schedule and appointment appropriateness.
• Provides triage services to walk-ins and over the phone.
• Participates in quality improvement and compliance activities.
