Job summary

Ohio County Healthcare in Hartford, KY is seeking an RN for preventive care within Population Health. You will coordinate cancer screening programs (cervical, colorectal, prostate), conduct tobacco cessation efforts, and review outreach with primary and specialty teams to ensure timely follow-up and documentation in the EMR.

This full-time position requires an RN license in Kentucky, BLS within 6 months, and 2 years of nursing experience in primary care or care coordination.

Qualifications

• RN licensure in Kentucky or eligible to obtain.
• 2 years’ nursing experience in primary care, care coordination, or related fields.
• Ability to obtain BLS within 6 months of hire.

Responsibilities

• Coordinate cancer screening programs (cervical, colorectal, prostate) and recall workflows.
• Review outreach, track results, and ensure timely follow-up with primary care and specialists.
• Educate patients on tobacco cessation options and coordinate therapies as ordered.
• Document outreach, assessments, results, and referrals in the EMR.
• Collaborate with Population Health Quality Lead and clinic staff to standardize workflows.
