Job summary

Hospice of the Valley is seeking an outbound bilingual Phone RN to contact patients after hospital discharge, assess experience, and provide discharge teaching. This role is based at our Central Clinical office in Phoenix and emphasizes telephone-based care and collaboration with the care team.

• Minimum two years of recent experience in medical, surgical or critical care nursing.

Position Profile

The outbound Bilingual Phone RN initiates phone calls to patients upon discharge from the hospital to determine the patient's experience and to provide discharge teaching. Scope of practice in this role is provided solely via telephone consultation. This position is base out of our Central Clinical office located in Phoenix.
