Elara Caring is seeking a Registered Nurse (RN) for Personal Care Services across Cape Cod, MA and surrounding communities. This role offers pay-per-visit compensation with no on-call, weekends, or holidays, and the chance to contribute to compassionate, patient-focused care.

You will lead assessments, develop care plans, supervise aides, and ensure regulatory compliance while supporting the mission of delivering high-quality, patient-centered care in the home setting.

Formación

• Graduate of an accredited School of Nursing.
• Current RN license in good standing in the state applying for.
• CPR Certification required.
• Minimum of one year of nursing experience; Home Care experience preferred.
• Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred.
• Strong clinical assessment, communication, and organizational skills.
• Excellent computer proficiency and documentation abilities.
• Ability to travel locally and regionally to support additional field offices as needed.

Responsabilidades

• Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery.
• Develop, review, and revise individualized plans of care based on patient needs.
• Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards.
• Monitor and report significant changes in patients’ physical and mental conditions.
• Educate caregivers, patients, and families using effective teaching and learning principles.
• Ensure all services meet State, Federal, contract, and licensure requirements.
• Conduct caregiver orientations and performance assessments as required.
• Evaluate home environments for safety risks and recommend solutions to improve patient safety.
• Maintain accurate clinical documentation, care plans, and service records.
• Communicate professionally with caregivers, clients, case managers, and leadership teams.
• Participate in staff meetings, in-services, and continuing education opportunities to support professional growth.
