Job summary

Elara Caring is seeking a Registered Nurse (RN) for Personal Care Services in Cape Cod, MA and surrounding communities. This role offers pay-per-visit with no on-call, no weekends, and no holidays, and emphasizes compassionate patient-centered care in the home setting.

The RN will conduct start-of-care and supervisory visits, develop care plans, supervise aides, monitor patient changes, and educate families while ensuring regulatory compliance and accurate documentation. Local travel is required.

Qualifications

• Graduate of an accredited School of Nursing
• Current RN license in good standing in the applying state
• CPR Certification required
• Minimum of one year nursing experience; Home Care experience preferred
• Knowledge of Medicare/Medicaid, home care benefits, and state regulations preferred
• Strong clinical assessment, communication, and organizational skills
• Excellent computer proficiency and documentation abilities
• Ability to multitask and adapt in a fast-paced healthcare environment
• Professional verbal and written communication skills
• Ability to motivate and lead caregivers in a positive and supportive manner
• Reliable personal transportation is required
• Ability to travel locally and regionally to support additional field offices as needed

Responsibilities

• Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery
• Develop, review, and revise individualized plans of care based on patient needs
• Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met
• Monitor and report significant changes in patients’ physical and mental conditions
• Educate caregivers, patients, and families using effective teaching and learning principles
• Ensure all services meet State, Federal, contract, and licensure requirements
• Conduct caregiver orientations and performance assessments as required
• Evaluate home environments for safety risks and recommend solutions to improve patient safety
• Maintain accurate clinical documentation, care plans, and service records
• Communicate professionally with caregivers, clients, case managers, and leadership teams
• Participate in staff meetings, in-services, and continuing education opportunities to support professional growth
