Job Summary

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, following physician's orders and complying with laws, regulations, and policies. The RN coordinates care plans with healthcare professionals, ensuring continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

  • Deliver high-quality clinical services within the scope of practice and infection control standards, according to the care plan, from admission to discharge.
  • Complete clinical nursing assessments as required by federal, state, and payer guidelines.
  • Verify patient eligibility and medical necessity for services.
  • Develop and update individualized care plans in collaboration with community providers.
  • Ensure care plan frequency and duration meet patient needs, revising with physician approval as necessary.

Experience Desired

Minimum one year of RN experience preferred.

License Requirements

  • Current RN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
  • Current CPR certification.

State Specific Requirements

  • TN: At least one year of clinical RN experience; this may be waived for RNs with recent LPN clinical experience. RN licensure must be unrestricted.
