Job summary

Shriners Children’s Spokane is seeking a full-time RN Care Manager to join our pediatric care team. You will perform assessments, planning, interventions, and follow-up for patients and families, coordinating care across the continuum and with the primary care medical home.

Ideal candidates hold a Bachelor in Nursing with WA RN license and at least 3 years of nursing experience; bilingual Spanish is a plus. Master’s in Nursing and Case Management Certification are preferred.

Qualifications

• Bachelor in Nursing with WA RN license required.
• At least 3 years nursing experience.
• Bilingual Spanish is a plus; Master’s in Nursing and Case Management Certification preferred.

Responsibilities

• Accountability for achieving optimal outcomes within standards of practice.
• Collaborates with patients, families, caregivers, and the health care team to coordinate care.
• Coordinates the care plan with the Primary Care/Medical Home and community resources.
• Advocates for patient health, safety, and rights.
