The Good Samaritan Patient Safety Team is a newly formed group dedicated to supporting operational teams across skilled nursing, assisted living, memory care, independent housing, home healthcare, and hospice services. We conduct event investigations using structured cause analysis techniques and champion the adoption of High Reliability principles—sensitivity to operations, reluctance to simplify, preoccupation with failure, deference to expertise, and commitment to resilience.

Recently, we successfully launched the SAFE event reporting system, powered by Riskonnect, across more than 300 post-acute care operations.

Summary

Actively participates in the overall safety program for Sanford by identifying potential risks within Post-Acute Health Services division facilities that impacts patient safety. Responsible for managing safety events related to residents, visitors, and employees within the patient safety evaluation system. Leads, facilitates, and advises key stakeholders, local/regional leadership, vice presidents and chiefs of medicine and nursing during analysis, deliberation and action planning of safe events and day to day resident safety concerns.

Job Description

Manages the day-to-day operation of patient safety programs and provides guidance to providers, clinical and leadership staff. Maintains systems with the organization to detect, monitor, prevent, organize, measure, investigate, report, and manage patient safety events and other indicators of potential patient harm. Aims to increase patient safety and reduce errors in post-acute healthcare processes. Conducts cause analysis of harm events and those with potential to cause harm. Collaborates with peers in Quality, Infection Prevention and Control, Compliance, Accreditation, Legal and others to identify and facilitate improvement initiatives based upon findings of cause analyses. Compiles and analyzes patient safety data for dissemination to operations and leadership. Prepares summaries for tracking, trending and analysis of event data. This position requires excellent organizational skills, attention to detail, written and verbal communication skills as well as a strong clinical background. Must have the ability to use independent critical thinking and analytical judgment in executing professional decisions. Problem solving skills are essential. Excellent interpersonal relationship skills are critical to facilitating groups and dealing with sensitive and confidential topics, and working towards conflict resolution in stressful situations. Must have comprehensive knowledge of regulatory, licensure and accreditation regulations related to patient safety.

Qualifications

Bachelor's degree in healthcare related field, Bachelor of Science in Nursing (BSN) preferred.

Five years clinical experience required. Knowledge of regulatory, licensure and accreditation regulations related to patient safety or risk management is desired.

Must currently hold an unencumbered registered nurse (RN) license with the State Board of Nursing. For Long term care they must maintain a certification/licensure as required by profession state(s) of practice and/or possess multi-state licensure privileges as required by position
