Job summary

Northwest Health - Porter is seeking a Patient Navigator - RN to guide patients and families through the continuum of care. You will coordinate services, educate patients, and resolve barriers to optimize treatment timelines.

In this role, you will collaborate with interdisciplinary teams, assess needs, and advocate for patients while maintaining communication with caregivers and providers to support a seamless, patient-centered experience.

Qualifications

• 3–5 years of clinical nursing experience required
• 1–3 years of experience in patient navigation, care coordination, or case management preferred
• RN license and Basic Life Support (BLS) certification required

Responsibilities

• Serve as primary contact for patients, families, and caregivers from diagnosis through end of treatment
• Assess medical, emotional, and socioeconomic needs and address barriers to care
• Provide patient education on treatment, side effects, and management strategies
• Facilitate timely, accurate communication among patients, families, and healthcare providers
• Collaborate with multidisciplinary teams to improve services and reduce treatment delays
• Coordinate referrals to social workers, financial counselors, dietitians, and community resources
• Assist patients with insurance-related tasks and financial assistance as needed
• Develop and update educational materials for patients and caregivers
• Track patient progress and adjust plans to evolving needs
• Maintain regular attendance and comply with policies and standards
