$45.75 - $68.61 an hour - Per Diem Days, Various shifts and hours per week
*Range includes per diem differential.
Boulder Community Health is seeking an RN Nurse Navigator for Oncology who is responsible for improving the quality of care, including the patient experience, of those who have a cancer diagnosis. Working in the three spheres of the patient, nursing practice, and the organization, the Nurse Navigator provides support and education and works to identify and remove barriers for cancer patients and their families who are newly diagnosed through all phases of the cancer journey. They guide the patient/family through the different phases of the oncology care continuum, from diagnosis through survivorship. The Nurse Navigator is charged with coordinating care and services at BCH and strengthening relationships with referral sources and community resources.

Qualifications
•Colorado RN License
•BSN preferred. Masters encouraged.
•CBCN and OCN certification preferred.
•2 years of oncology nursing experience.
Responsibilities
•Conducts comprehensive, holistic wellness and illness assessments using known or innovative evidence-based techniques, tools and methods.
•Identifies potential risks to patient safety, autonomy, and quality of care based on assessments across the patient, nurse and system spheres of influence.
•Synthesizes assessment data, advance knowledge, and experience, using critical thinking and clinical judgement to formulate a plan of care and intervention.
•Develops evidence-based clinical interventions and systems to achieve defined patient and system outcomes.
•Uses advanced communication skills within therapeutic relationships to improve patient outcomes.
•Provides direct care to patients undergoing cancer care based on the needs of the patient.
•Provides leadership for collaborative, evidence-based revisions of diagnoses and plans of care, to improve patient outcomes.
•Provides education (appropriate to the patient’s health literacy level, learning needs, readiness to learn and cultural values and beliefs) for patients and their families.
•Identifies, develops and maintains reference materials for patients and families.
•Coaches patients and families to help them navigate the healthcare system.
•Builds relationships with medical and radiation oncologists, surgeons, specialists, primary care physicians and other referral sources in the community, increasing awareness of programs and •utilization of services offered.
•Determines need for patient /family support groups and integrative services. Works with available resources to meet those needs.
