Job summary

Northwestern Medicine in Illinois is seeking a Patient Flow Coordinator, RN (PFC, RN) to join the ICC team. The role delivers direct patient care and consultation, applying in-depth nursing knowledge to promote holistic care and coordination with the healthcare team.

Shifts are 12 hours with weekend and holiday requirements. A BSN and Illinois RN license are required; emergency department experience is preferred. This role emphasizes patient safety, documentation, and collaboration.

Qualifications

• The candidate must have a BSN and an active Illinois RN license.
• BLS certification is required.
• Minimum two years of RN experience; emergency department setting preferred.

Responsibilities

• Provide direct patient care and consulting within a fast-paced ICU/ICC environment.
• Perform nursing assessments, document in the medical record, and ensure patient confidentiality.
• Coordinate patient care with other clinical and administrative departments.
• Triage and respond to urgent/emergent medical situations per policy.
• Serve as a resource for staff and physicians and mentor team members.
