Qualifications

Responsibilities

• Provide direct patient care to a diverse population, including assessments, medications, treatments, and procedures.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Monitor and document patient progress, identifying and escalating concerns as necessary.
• Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions.
• Collaborate with interdisciplinary teams to develop and implement evidenced-based practices.
• Maintain accurate and up-to-date records, adhering to HIPAA and facility policies.
• Communicate effectively with patients, families, and healthcare teams, promoting a culture of transparency and respect.
