Qualifications

• Nursing Diploma or Associate's degree required.
• RN licensure/registration required.
• Preferred Bachelor's degree in nursing.

Responsibilities

• Provides direct patient care including assessment, planning, implementation and evaluation.
• Educates patients and families on illness, treatment, home care needs and medication administration.
• Participates in safety, quality and regulatory activities and professional development.
• Develops and documents individualized plans of care with interprofessional team.
• Communicates plans of care across the continuum of care.
