Job summary

Qualifications

- License in good standing; no abuse/neglect findings
- Ability to collaborate with interdisciplinary care team
Responsibilities
- Deliver quality nursing care focused on outcomes using the nursing process
- Identify needs and contribute to discharge planning and follow-up
- Develop individualized care plans with residents and care team
- Implement care plans effectively and cost-efficiently with resident-centered outcomes
- Collaborate with physicians and other health professionals sharing changes in care plans
- Deliver and maintain optimal resident care and comfort with current nursing practices
