Job summary

You will collaborate with interdisciplinary teams, implement evidence-based practices, and participate in quality improvement initiatives to enhance patient outcomes.

Qualifications

• Graduate of an accredited school of nursing required.
• Patient- and Family-Centered Practice knowledge of how to convey respect and collaborate with each patient, family and/or designated care partner in care and care planning.
• Cultural Awareness knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
• Effective Communication understanding of communication concepts, tools and techniques, ability to transmit, receive, and interpret ideas, information, and needs meriting partnership with patients and families.
• Patient Assessment and Care knowledge of abilities to assess ailments and implement a treatment plan.
• Medication Administration and Prescription knowledge of medications and administration.
• Discharge Planning knowledge of post-discharge needs and arranging follow-up care.
• Health Information Documentation knowledge of documentation methods and medical history recording.
• BLS & ACLS - American Heart Association or Red Cross.
• RN - Licensed Registered Nurse_NJ - State of New Jersey

Responsibilities

• Collaborates with patients, families, and staff to deliver care aligned with Jefferson’s values.
• Implements Evidence-Based Practice to improve outcomes and safety.
• Educates patients and families on conditions, treatments, and self-care.
• Ensures safety by following nursing protocols and reducing risk of harm.
• Participates in hospital and practice committees, including quality improvement.
• Uses information systems to document care and monitor outcomes.
