SourcePro Search, LLC is seeking a Nurse Leader (RN) to guide our home health team and advance patient care across the community. In this role you will empower clinical staff, foster collaboration, and uphold safety and quality.You will champion operational excellence, ensure regulatory compliance, and influence patient outcomes while supporting professional growth for team members. This is a full-time, on-site leadership position in California.

Qualifications

• RN licensed in the practicing state.
• Minimum 5 years as an RN, with home health experience.
• Minimum 2 years in management or supervision in home health (preferred).

Responsibilities

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.

Qualifications:

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
