The Keystone Distinction Group is seeking a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. You will empower clinical staff to foster a collaborative environment where care teams thrive and deliver compassionate home health services.

You will champion operational excellence, ensure regulatory compliance, and influence patient outcomes while supporting professional growth for your team and maintaining the highest clinical quality and safety

Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements

Responsibilities

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols.
