Job summary

Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state (CA).
• 5 yrs RN experience with home care setting.
• 2 yrs management or supervisory experience in home health (preferred).
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with standards and regulations.
• Plan, coordinate, and monitor patient care for high-quality outcomes.
• Supervise, support, and coach clinical personnel to maintain care excellence.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team.
