As an RN Patient Care Manager, you can expect:

Responsibilities

• Receive referrals and ensure appropriate clinician and/or therapist assignments for timely patient evaluation, signing off after authorization, and plotting start of care (SOC) visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Continuously assess patient needs; ensure care is individualized, appropriate, meets home health eligibility criteria, and aligns with physician orders.
• Oversee and update the individualized patient plan of care as appropriate.
• Manage and document communications from physicians, clinicians, patients, and referral sources. Communicate patient updates and new orders to clinicians, and document using coordination notes as needed.

Education and Experience

• Current RN licensure in the state of Arizona
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle
• At least one year of professional home health nursing experience

Salary: $75,000 - $80,000 annually, based on experience, plus annual incentives
