Job Description

• Helps patients access necessary healthcare services and resources, including financial assistance and community support.
• Mediates and resolves conflicts between patients and healthcare providers or institutions.
• Collects and documents patient complaints and grievances, following up to ensure resolution.
• Collaborates with healthcare teams to improve patient care and satisfaction.
• Advocates for patient safety and quality care, identifying and addressing potential issues.
• Participates in performance improvement initiatives and policy development to enhance patient advocacy services.
• Provides emotional support and empathy to patients and families during difficult times.
• Maintains confidentiality and adheres to ethical standards in all interactions.
• Provides support and guidance to patients and their families, helping them navigate the healthcare system.
• Communicates effectively with patients, families, and healthcare providers to ensure patient needs and concerns are addressed.
• Educates patients about their rights and responsibilities within the healthcare system.
• Assists patients in understanding their medical conditions, treatment options, and care plans.
• Other duties as assigned.
