We're hiring for a Part-Time or Per Diem Home Health Registered Nurse to join our passionate team in Fairview Heights, IL! This role will serve patients in Fairview Heights and surrounding areas.

Salary: $73,585 - $89,936 (per visit position)

Responsibilities

• Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
• Conduct initial and/or comprehensive nursing evaluations, determine primary focus of care, develop the plan of care within state guidelines, and submit accurate, complete, and timely documentation.
• Regularly evaluate and re-evaluate (at least every 60 days or as condition changes) the patient's nursing needs.
• Perform comprehensive assessments at designated times and develop appropriate plans of care in collaboration with physicians.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Initiate, develop, implement, and revise the plan of care collaboratively with physicians and other healthcare professionals.

Education and Experience

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
