Job summary

Scotland Memorial Hospital in Laurinburg, NC seeks an RN Service Line Coordinator for the Orthopedic program. You will guide patients from referral through recovery, coordinate tests, clearances, and follow-ups, and educate families on expectations and care plans.

Qualifications

• Graduate of an accredited nursing program.
• Current NC or Multistate RN license; BSN, BLS, ACLS.
• Preferred ONC certification within 1 year of hire.

Responsibilities

• Act as the primary point of contact for orthopedic patients from referral to recovery.
• Coordinate appointments, diagnostics, pre-operative clearances, and post-operative follow-ups.
• Ensure timely movement of patients through the care continuum, minimizing delays.
• Educate patients and families on procedures, expectations, recovery plans, and care pathways.
• Identify and address barriers to care (transportation, financial, social determinants of health).
• Support implementation and adherence to standardized orthopedic pathways.
• Monitor patient progress across the continuum and ensure compliance with protocols.
• Facilitate prehabilitation and post-acute care coordination.
• Partner with orthopedic surgeons, APPs, and multidisciplinary teams.
• Coordinate interdisciplinary communication to ensure seamless care transitions.
• Support physician practices with scheduling optimization and case progression.
• Promote patient engagement tools and monitor satisfaction.
• Track key service line metrics and identify opportunities for process improvement.
• Maintain accurate documentation in the EMR and assist with reporting.
