Current LVN/LPN License Required

REMOTE: MUST RESIDE IN CALIFORNIA OR PST TIME ZONE

Schedule: 8:00 AM to 7:00 PM Saturday and Sunday, with 2 additional shifts during the week to be discussed during interview

Pay Range: $32-$38 per hour

What You Need to Know

The Clinical Intake/Admissions Coordinator (RN or LVN/LPN) is responsible for the timely acceptance or decline of referrals based on the criteria set by the Agency. This position's territory may encompass one or more states and may change with the needs of the Company. The Coordinator establishes and maintains communication with clients, family members, and referral sources. This role partners with agency staff, referral sources, and sales to achieve optimal patient satisfaction and outcomes. The Coordinator facilitates obtaining all necessary information and documentation to complete a referral through follow-up with the Agency, Referral Source, and Client.

Qualifications

Be the Best Clinical Intake/Admissions Coordinator You Can Be

If you meet these qualifications, we want to meet you!

  • Licensed RN or LVN in practicing state
  • 2+ years of relevant experience, including understanding of discharge processes and intake protocols
  • Minimum of one year experience in home care, hospice, or personal care services (preferred)
  • Prior use of Electronic Medical Records (EMR)
  • Proficiency with clinical applications and MS Office
  • Experience dealing with various payors like Medicare, Medicaid, and commercial payers (preferred)
  • Ability to adapt in a fast-paced environment
